Are you leaving Illumina? We wish you well and want to help you understand what will happen to your company benefits. Most will end on your last day of employment, but there are important details you’ll want to know.
Many of the notifications/forms noted below will be mailed directly to your home address. Please confirm that your home address and email are correct in Workday before your last day.
Illumina will send your separation date to Fidelity. You will receive information about your distribution options at your home address.
Benefits end on the last day of the month in which your employment ends.
You may elect to continue coverage under COBRA. Vita, our COBRA administrator, will send you information to your home address within 14 days of your last day of employment. If you have a personal email address in Workday, you will also receive the COBRA information via email. Typically, this email is sent within 5-7 days following your last day of employment.
Benefits end on your last day of employment.
You must submit all claims for reimbursement from your Health Care FSA within 90 days following your separation date.
Important: Only those expenses incurred on or prior to your last day of employment will be eligible for reimbursement.
You may elect to continue your Health Care FSA on a post-tax basis through the end of the calendar year. See your COBRA packet for details.
You can continue to incur eligible claims through the end of the year following your last day of employment. Claims must be filed by March 31 of the following year.
Your HSA contributions end with your final paycheck. Any existing account balance is yours to keep. You may continue to access and use your HSA funds for eligible expenses after you leave Illumina.
HealthEquity will charge a monthly maintenance fee to your account once you leave Illumina. You may have the option to roll over your HSA balance to another employer HSA or a personal HSA. Contact HealthEquity for more information.

You will receive information directly from the insurer (NY Life) about the option to port your coverage or convert to an individual policy.

You and your eligible dependents will receive a COBRA Qualifying Event notice from Vita within 14 days of your separation date.
You have 60 days from the date the notice was mailed to enroll in COBRA to continue your medical, dental, vision, Lyra, and/or Health Care FSA coverage.
If you continue your coverage, you will pay your premiums to Vita.
