Benefits are available if you are enrolled in an Illumina medical plan. This includes the Traditional PPO, the Health Savings PPO, Kaiser HMO (CA) and Quartz HMO (WI).
Infertility benefits are administered by Progyny and bundled with medical plan benefits. You’re responsible for any deductible and coinsurance for covered services, including your initial consultation and diagnostics, medication, and fertility treatment.
Smart CyclesProgyny benefits are structured around “Smart Cycles,” which bundle services such as in vitro fertilization (IVF), intrauterine insemination (IUI), egg freezing, donor sperm/egg coordination, genetic testing, embryo freezing and storage. Illumina provides 2 initial consultations per year and 2 Smart Cycles per family per lifetime. If the first two are not successful, you have access to an additional Smart Cycle. You’ll be matched with a dedicated Patient Care Advocate (PCA) who will guide you throughout your journey. They can help schedule appointments, answer questions, and provide emotional support.
Progyny’s benefit also includes fertility medication and pharmacy coordination, so you get exactly what you need when you need it.
Progyny provides support for all family-building journeys, including single parents by choice and LGBTQ+ individuals and couples.
If you or your spouse/domestic partner is facing fertility concerns and considering IVF, with or without PGT, or if one of you has recently become pregnant, you may be eligible for company-sponsored financial support of certain genetic testing through the Reproductive Health Reimbursement Program.
Since employees enrolled in an Illumina medical plan are already provided coverage for these tests, the reimbursement benefits available under the Reproductive Health program are only available to employees or dependents who do not have coverage for these tests under another medical plan. The following services are covered with financial support from the Reproductive Health program:
Employees and their spouses/domestic partners who are not enrolled in an Illumina medical plan and do not have fertility benefits through other coverage.
Complete and sign the Employee Reproductive Financial Support form — available from the Insider.
Meet with the ordering physician who signs the Financial Support form and orders PGT or NIPT.
Submit the signed Financial Support form to the AMR Benefits Team using a ServiceNow ticket.
The Benefits team reviews and processes a one-time payment through Workday. Payment will be in USD (income taxes apply).
