Healthy smiles start with regular care. Illumina’s Dental PPO plan offers coverage for preventive care and basic and major dental services, including orthodontia.
The Dental PPO plan is administered by Guardian.
The plan gives you the freedom to seek services from any dentist, but when you visit in-network (DentalGuard Preferred) providers, you receive a greater benefit because Guardian Dental providers offer discounted rates.
If your total paid claims are less than $800, you’re eligible for a rollover.
Guardian adds $400 to your annual benefit for next year. If you only use in-network providers, you’ll get $600 instead.
Your rollover account can grow to a maximum of $1,500 to help pay for future care.
