Illumina automatically provides Basic Life and AD&D Insurance at no cost to you.
You can purchase additional Supplemental Life and AD&D Insurance for yourself, your spouse/domestic partner, and children as a new hire, during Open Enrollment, or after a qualifying life event.
Basic Life and AD&D Insurance covers two times your annual salary to a maximum of $1,000,000.
For Supplemental Life and AD&D Insurance, the amount you, your spouse/domestic partner, and children can elect varies.
Benefits are paid to your beneficiary.
Your benefit is equal to 2x your annual salary, up to $1,000,000. This benefit is taxable per IRS requirements. The benefit amount reduces starting at Age 65. See the Group Life Insurance Certificate for more details.
As a new hire, you can purchase coverage up to 5x your annual salary or $300,000 (whichever is less).
As an existing employee, you can purchase coverage up to 5x your annual salary or $250,000 (whichever is less) during Open Enrollment or if you have a Qualifying Life Event.
You must submit evidence of good health to elect coverage above the Guaranteed Issued amount of $150,000. You’ll receive a letter from New York Life with the option to complete an online form, or complete and return a paper form. Your coverage will default to the Guaranteed Issue amount until the additional coverage is approved.
As a new hire, you can purchase coverage up to $25,000 for your spouse/domestic partner if you elect supplemental life and AD&D coverage for yourself.
As an existing employee, you can purchase coverage in increments of $5,000 up to $100,000 if you elect supplemental life and AD&D coverage for yourself. You must submit evidence of good health to elect coverage above the Guaranteed Issue amount of $25,000.

Premiums for Supplemental Life and AD&D insurance coverage depend on the benefit amount you select and are deducted from your paycheck after taxes. You will see the amounts when you enroll through Workday.
